I understand I must allow at least 10 working days to process my request. I understand that providing incorrect or incomplete information may delay the processing of my sacramental records, and that processing may take longer during busy times, such as peak sacrament registration periods, the Christmas season, or other busy periods for the church office.

I understand certificates not claimed within 60 calendar days will be destroyed and a new certificate request must be submitted.

I understand that there is a $10 fee for each certificate request submitted

The $10 fee can be paid via this page or with cash, card, or Apple Pay in person in the church office.
